**Activity Description:**

I am the legal guardian of this child(ren) and acknowledge that my child(ren) will be participating in classes or events organized by “The Other Hearth LLC” at privately held locations in Santa Ynez, CA, a park, hiking trail, or other previously agreed upon location.

**Release, Assumption of Risk, and Waiver of Liability:**

I understand that participation in The Other Hearth workshop, classes, and camp activities (including outdoor activities and crafts) involves inherent risks of injury, illness, disability, or death, whether caused by my actions, the actions of others, or conditions of the location or equipment. I freely and voluntarily assume all such risks, both known and unknown, that are reasonably related to these activities. To the fullest extent permitted by California law, I release and waive any claims against The Other Hearth, any other businesses associated in hosting its activities, and their officers, directors, employees, volunteers, and agents for ordinary negligence resulting in injury, illness, death, or property damage arising from participation in the camp.  I acknowledge that my child(ren) is(are) in good health and physically able to participate.

I understand that this release does not apply to claims arising from gross negligence, recklessness, or intentional misconduct, which cannot be waived under California law.

 

**Medical Consent**

As the parent/guardian, I authorize The Other Hearth staff to provide routine first aid for minor injuries, including cuts, scrapes, bumps, bruises, and insect stings.

**Indemnification:**

​

I agree to indemnify and hold harmless “The Other Hearth LLC” and the property and its owners at which the classes are held from any claims, damages, or expenses arising out of my child’s participation.

**Acknowledgment of Understanding:**

I have carefully read this waiver and release of liability and fully understand its terms. I acknowledge that I am voluntarily signing this agreement and understand that by doing so, I am waiving, releasing, and discharging any and all claims, causes of action, or demands of any kind whatsoever, in law or in equity, that I or my child may have now or in the future against “The Other Hearth LLC”, its owners, directors, officers, employees, volunteers, agents, and affiliates, arising out of or related to participation in activities, including claims arising from the ordinary negligence of the released parties. I further acknowledge that I am assuming all risks associated with participation and expressly intend this release to be as broad and inclusive as permitted by the laws of the State of California.
