**Activity Description:**

I am the legal guardian of this child(ren) and acknowledge that my child(ren) will be participating in events organized by “The Other Hearth LLC” at 3118 Mission Dr. Santa Ynez, CA 93460, a park, hiking trail, or other previously agreed upon location.

**Assumption of Risk:**

​

I understand that participation in these activities involves inherent risks, including but not limited to physical injury, accidents, or illness. I acknowledge that my child(ren) is(are) in good health and physically able to participate.

**Release of Liability:**

​

In consideration for allowing my child to participate, I, on behalf of myself and my child, hereby release, waive, and discharge “The Other Hearth LLC”,  property owners of 3118 Mission Dr. Santa Ynez, their officers, agents, employees, and volunteers from any and all liabilities, claims, demands, or causes of action that may arise from participation in these classes.

**Indemnification:**

I agree to indemnify and hold harmless “The Other Hearth LLC” and the property and its owners at which the classes are held from any claims, damages, or expenses arising out of my child’s participation.

**Acknowledgment of Understanding:**

I have read this waiver and understand its terms. I am aware that by signing this waiver, I am giving up certain rights on behalf of myself and my child.

My submission of the registration form will constitute my signature and agreement of the above
